Vertebrogenic lumbalgia - treatment of low back pain

Vertebrogenic lumbalgia has the following clinical picture. During the illness, there is a mild or moderate pain syndrome in the lumbar region with signs of reflex tonic protection of the vertebrae. The pain is caused by various exogenous effects: lumbar injuries, systematic physical overwork or excessive efforts, sharp gustatory movements, work in an uncomfortable position, hypothermia, acute respiratory diseases, exacerbation of foci of focal infections.

Studies show that the appearance of pain often coincides with an active increase in weight and height in children and adolescents (up to 10 cm per year), which contribute to the instability of the development of the spine. About a fifth of patients have whitish transverse strips in the sacro-lumbar region, which indirectly confirm muscle and bone inadequacy during the rapid growth of the skeleton.

With the diagnosis of vertebral lumbargia, in most cases, the patient's condition is determined by the painful condition of the spinous processes and paraverterbial points, about a third of patients experience mild or moderate tension symptoms, Laseg's symptom often appears , and in rare cases symptoms of Matskevich, Wasserman, Dezherin, Neri are possible. During an illness, there are usually no changes in the peristal-tendon reflexes and motor disorders.

In case of a disease, vertebrogenic lumbargia on the x-rays of the spine made in half the cases determine the flattening of the lumbar lumbar region and a large number of Schmorl hernias that are large and often located in the anterior part of the site of the upper lumbar vertebrae, causing their deformation. Lower-lumbar spinal discs often prolapse into the posterior half of their sites. Sometimes a mild scoliosis of the lumbar region is found, which is combined with the smoothness of the lumbar lordosis. The height of the intervertebral gaps decreases very rarely. The above described changes in two-thirds of patients are combined with congenital abnormalities of the development of the sacro-lumbar spine (transitional lumbosacral vertebra, vertebra arches).

Vertebrogenic lumbalgia is characterized by lumbar lumbago and acute lumbar pain, which occurs when tilting, flexing, torso turns, lifting weights. Thus there is a tension of the muscles of the waist. The disease can occur in a long chronic form, with alternating relapses and remissions. Even with the elimination of pain syndrome, the feeling of discomfort in the lumbar region remains for a long time.

In children, the disease is much less common than in adults, and can begin at the age of 8 years.

In chronic long-term course of the disease with a persistent pain syndrome present and gross muscular-tonic disorders, the ailment can lead to disability.

Vertebrogenic lumbalgia: treatment

During the acute period, the main task of treatment is to stop the pain syndrome. For this, it is necessary to provide the spine with a resting state, limiting the motor regime for a week. It is recommended to comply with bed rest. To get up is allowed only in extreme necessity, wearing an immobilizing belt or corset. The patient should be on an orthopedic mattress or on an ordinary with a shield.

Therapy includes the use of anti-inflammatory non-steroid drugs, anesthetics and analgesics. Depending on the level of vasomotor disorders and the presence of concomitant vascular diseases, vasodilators are prescribed . With chronic pain, psychotropic drugs are prescribed.
Prescribe fizeo-, manual and kinesitherapy.
