In 1994, it was decided to allocate the assigned amount from the state budget for the implementation of highly qualified medical care - the VMP (which it will be described in more detail below). The first institution funded for innovative operations was the All-Russian Research and Production Cardiology Center. After a while, new technologies in medicine became available for other clinics. Before the Federal Law "On the Fundamentals of Health Care of Citizens in the Russian Federation" was issued, the Ministry of Health and Social Formation annually approved the referral of patients for the provision of high-quality medical care. In 2012, a new order was legislatively adopted. From the moment of entry into force, its effect is unlimited. Consider how to get into institutions that use new technologies in medicine. Direction. General information

A ticket for the provision of VMP is a kind of primary documentation. It includes information used to organize accounting and control of each direction. It is filled in by the health management authority (HMO) individually after submitting the documents to the commission for the selection of patients at the VMP (what is this commission, will be explained below). The documentation should contain records of the medical institution. In the process of filling out the referral for medical care , only Russian can be used. The document includes the passport part. To it carry sections Т, М, И, У, and 6 sections which correspond to the periods of a direction on rendering ВМП. Next, we'll look at how the document is drawn up.

How do you fill in the direction for the VMP?

What is this document, we have already found out. Then some rules for filling out will be given. Computer technologies in medicine are used not only in the treatment of pathologies or their diagnosis. Also, their use allows you to quickly draw up documentation of various kinds. Its dark fields are made only in electronic form at the stages of sending the patient's documentation to a correspondence consultation with a medical institution and providing information about the decision taken from him. If the medical institution finds it necessary to resort to face-to-face consultation or prescribe the date of the alleged hospitalization, the health authority issues a paper ticket that includes itself and the passport part. When connected to a common information system, the document number is automatically generated. In other cases, it is established in accordance with the instructions given in the "Reporting Handbook".

Passport part

The "T" section contains the details of the direction. In point T.1 the number, month and year of submission of documents, pre-filled by the medical institution, to the commission of the healthcare management body is indicated. Requisites of MU are filled in section M. Items T.2, T.3, T.4 contain the information indicated in the figures, according to the comments in each of them. Point T.5 reports information in figures about the body that gave the direction. Here is an example of such digital notations:

If in clause T.3 the figures 1 or 2 are indicated, information on patients is entered into the forms of information about the provision to citizens of the VMP. Section "M" contains the full name and requisites of the medical institution to which the patient was referred. In the sections "AND" and "U" the patient's data - surname, name, patronymic, date of birth and place of registration, sex corresponding to the passport are indicated. In item I.4, the insurance number of his personal account is indicated, which must correspond to the document for receiving a number of social services. Item I.8 contains the code of the document type, established by a single classification. In items V.3, Y.5, Y.6, the information is provided with the corresponding figures. Paragraph V.4 is completed for those categories of patients who enjoy benefits. In the line, zeros are inserted until the first significant digit appears. In clause V.7 and V.8, the patient information is indicated by the sign V.

Stage number 1. Health authority

If there is a need to conduct additional examinations or re-arrange the documents, clauses 1.1 and 1.2 are marked with the V mark, and patient information is entered in the "waiting list". When the patient provides the necessary information, in clauses 1.3 and 1.4, the digits indicate the code and date of the decision, respectively. If the refusal to provide assistance has come, in 1.3, the reason code for refusal should be indicated. The coupon of the VMP is closed at the same time. Paragraph 1.5 is accepted to be filled in accordance with the rules of the International Statistical Classification of Diseases and Problems, guided by information on the severity, stages, and course of the disease. In item 1.6 the codes corresponding to the "Directory to the reporting" are indicated. Paragraph 1.8 discloses the full name of the medical institution, which coincides with its name in the registration records. In clause 1.9 the code of the region is indicated, where the institution that sends information about the patient is located. This information is necessary for making the correct decision and is presented in the form filled in accordance with the established rules of the direction. It specifies a detailed extract from the medical history, containing the opinion of the responsible chief specialist explaining the need to provide high-quality medical care. This section also contains information about clinical, laboratory and other studies and diagnostics that are required for a particular type of illness. This information should be no more than a month old. In paragraph 1.10, the date of sending documents using e-mail is indicated. The medical institution must send a confirmation of receipt of these documents.

Stage number 2

In clause 2.1, it is indicated when a voucher and medical documentation for the patient was received by e-mail from the health authority. The date must correspond to the one registered in the electronic notification register. If there is a need for an additional examination of the patient, re-registration of the documentation by the commission of the medical institution, in points 2.2 and 2.3 a note V is made. Information on the decision is addressed to the health management body with an explanation of the necessary types of additional surveys and instructions for processing the documents. If the commission of the institution has decided to provide the patient with the ILC, paragraph 2.4 indicates the figure 1. In this case, the date of the proposed hospitalization is set in clause 2.7. Par. 2.6 must contain the service type code. Data about the patient are placed in the "turn on the VMP". After that, the information is sent to the health management body in electronic form. If a decision was made to refuse to receive a VMP, paragraph 2.4 should contain the reason for the refusal (indicated in figures).

Stage No. 3

With a positive decision of the Commission of the medical institution on hospitalization, the health management authority sends the patient to the place where the treatment will be administered. In this case, in 3.2 the date of the decision is put. Paragraph 3.3 contains information on the date of issue to the patient of the coupon in the VMP, which is made in paper form and filled in according to the data of the second stage, as well as accompanying documents. When specifying the date, the time spent on travel is also taken into account. In clauses 3.4 and 3.5, the day of issuing travel coupons to the place of recovery by the Social Insurance Fund of the Russian Federation and their number, respectively, is indicated. In the case of a limitation of the patient's ability to work in the third degree, a note on his accompaniment is made in point 3.6. After filling in section 3, the coupon-sending is sent to the medical institution in electronic form. Further, the document with the signature of the health management authority and its seal is issued to the citizen in paper form.

Stage No. 4

In clause 4.1 the physician specifies the date of the patient's referral to the medical institution with the provision of a coupon for the provision of a VMP, issued in a certain order, an extract from the documentation that contains the conclusion of the chief specialist explaining the need for the provision of services. Also in the direction should be indicated the results of the diagnostic procedures required for this disease, with a prescription of less than a month. If the commission of the institution has authorized the receipt of the VMP, in item 4.2 the figure 1 shall be indicated, in clause 4.5 the day in which the decision was made, and in clause 4.6 the date of hospitalization shall be entered. If it was refused, then in clause 4.4 with the use of certain figures the basis for it is indicated. Then the information is sent to the health authority, and the document is issued to the patient.

Stage number 5

Paragraph 5.1 should contain the date of the issuance of the citizen to whom the VMP was provided in accordance with clause 22 of Form No. 066 / o-02. In clause 5.2 the figures indicate the result of the provision of the VMP. Paragraph 5.3 provides information on the forms, severity, the nature of the course of the disease, which is taken from documents in accordance with the International Statistical Classification of Diseases and Health Problems. In clause 5.4 the codes of standards of rendered medical care are given, which are given in the "Reporting Handbook". Item 5.5 contains information on the result of hospitalization in figures. Paragraph 5.6 indicates the planned date for a second referral to a health facility, if required. Then in the direction - a form filled in paper form - the signature and seal of the head of this organization is put, after which it is given to the patient's hands. On the day of receipt, the electronic form of the document is sent to the health authority.

Quota of the VMP

Previously, it was issued after appealing to the department, ministry or commission directly. To do this, it was necessary to have an extract from the documents containing the recommendations, copies of the passport, the pension certificate and the MHI policy. The health authority included a special commission, which selected patients to provide them with VMP. For 10 days she made a decision. On December 28, 2011, the Order of the Ministry of Social Development No. 1689n was approved. In this regard, the turn on the VMP changed a little. The decision from that moment is made by the commission of the health authority of the subject of Russia on the choice of patients. Now the selection of citizens and their referral to this commission are carried out by medical institutions, in which patients were observed and treated. The appointment is issued on the recommendation of the attending physician, based on an extract from the patient's medical records. This statement should include the diagnosis, information about the patient's health status, the diagnosis code in accordance with the International Statistical Classification of Diseases and Health Problems, the examinations and treatment performed, the reasons for the forced delivery of the VMP. In addition, the results are attached to the results of all types of diagnoses that are carried out, corresponding to the specificity of the disease, which led to a definite diagnosis. The medical commission analyzes the information for three days and makes a decision consisting of permission or refusal to send documentation to the Commission of the subject of the Russian Federation. The adopted resolution is sent out before the dispatch by the protocol. The basis for the decision is medical indications for the provision of services, taking into account the list of species.

In the event that the medical commission decides to send the patient's documentation to the Commission of the subject of the Russian Federation, it must within three working days form and send a set of documents to the health authorities. A citizen who is provided with assistance has the right to demand a protocol of the medical commission's decision and an extract from his medical card in his hands, as he can take them to the health authorities directly. In the event of refusal to provide VMP, the patient should be given a protocol with the decision taken. It will indicate the grounds for refusal and an extract from the documentation. Cities of federal significance

How does the citizens go to the VMP? Moscow, for example, as a city of federal significance, has special conditions. Selection of patients is carried out in the institutions of the state system of the city and in federal organizations that are subordinated to the Ministry of Health of the Russian Federation. To date, 36 hospitals in Moscow provide more than 80 types of VMP. According to statistics, every year more than 58 000 medical and diagnostic procedures are provided, using advanced development. Many city hospitals have departments of traumatology, where about 3.5 thousand surgical interventions for endoprosthetics of large joints are carried out.
