The practice diary contains such information:

  1. Full name of the student. ________
  2. The name of practice. For example: "Training and production practice with a separation from the educational process (the total number of hours - 56)."
  3. Period of passage. ("The training and production practice lasted from ___ to ___").
  4. Place of passing the practice (the full official name of the organization where the student was practicing).
  5. Leader of practice from the organization. Name, position.
  6. Head of practice from the university. Name, position.

If there are official forms, then here, according to the requirements, the seal of the organization, the signature of the managers and the date can be put.

Then follows the main part, which is convenient to present in the form of a table.
