Amlodipine instruction

The drug "Amlodipine" is probably familiar to many not first-hand. Basically, this drug is prescribed for patients with angina pectoris. Let us consider in more detail the preparation "Amlodipine", the instruction of which contains the following information.

Composition. One tablet (5 mg) contains the active substance amylodipine besylate. As auxiliary substances are used milk sugar, potato starch, kollidon, microcrystalline cellulose, magnesium stearate.

General description. "Amlodipine" tablets are white or almost white with a double-sided facet. Sometimes there may be light marbles.

Pharmacodynamics. The dihydropyridine derivative blocks the calcium "slow" channels of the second generation, has a hypothetical and antianginal effect. It binds to dihydropyridine receptors and performs blockade of calcium channels, thereby reducing the transmembrane transition into the cell of calcium ions (mostly in the smooth muscle of the vessel walls than in myocytes of the heart). Antianginal action is caused by the expansion of arteries and arterioles that are in the periphery: when angina decreases, it reduces severe myocardial ischemia, dilates the peripheral arteries, while reducing the overall vascular peripheral resistance, in addition helps reduce preload on the heart, reduces the need for a heart in oxygen. In patients with angina pectoris, the daily single dose increases the duration of the discharge, slows the development of ischemia and angina pectoris, plus reduces the frequency of angina attacks and the need for nitroglycerin. Thus, patients with angina pectoris need " Amlodipine", the instruction is proof of this.

Pharmacokinetics . After oral administration, the drug is slowly absorbed from the digestive tract. The absolute average bioavailability is 64%, the maximum is concentrated in the serum after 6-9 hours. Stable equilibrium in the blood is observed on the 7th day. Food does not affect the absorption of amlodipine. In elderly patients (older than 65 years), excretion of this drug is slower than in young patients, but this difference is clinically insignificant.

"Amlodipine", the instruction of which contains all the necessary information, has the following indications for use: with arterial hypertension (combination with other similar drugs or monotherapy), angina pectoris, vasospastic angina (Prinzmetal).

Contraindications.

- high sensitivity to the active substance and other dihydropyridine derivatives;

- arterial hypotension in pronounced form;

- cardiogenic shock, collapse;

- unstable angina (exception - angina of Prinzmetal);

- Lactation and pregnancy;

- for persons under 18 years of age, safety and effect have not been established.

Dosage and route of administration . Accept the "Amlodipine" instruction as follows: inside, a minimum dose of 5 mg, a maximum of 10 mg, once a day. With arterial hypertension, the dose to maintain may be 5 mg per day. When angina pectoris take once 5-10 mg per day. Thin patients, as well as low patients and patients with impaired liver function, are prescribed 2.5 mg.

Special instructions. During the course of treatment with the drug "Amlodipine" it is necessary to monitor the body weight and consumption of such substance as sodium, sometimes a special diet is required. It is important to monitor hygiene of the mouth and teeth, visit the dentist regularly (to avoid pain and gum bleeding). When increasing the dose in the elderly requires careful monitoring of them. The effect of the drug on the ability to drive a car there is no data. But, it should be borne in mind that in some people it can cause mild dizziness and drowsiness.
